Fold base-react into @hanzo/base/react; fix Dart package name to hanzo_base

The standalone @hanzoai/base-react (never published to npm) duplicated
React bindings that already ship inside @hanzo/base as the @hanzo/base/react
subpath export (a superset). Retire it: replace sdk/base-react with a Moved
pointer, so the SDK consolidation no longer carries a package that imported
the removed file:../base-js. Update the design doc's example imports to the
one true entry point @hanzo/base/react.

Also correct the Dart client name in the Moved pointers: pub.dev 'hanzo'
and 'base' are taken by unrelated packages, so the Dart package is hanzo_base
(hanzo-dart/base).

The React bindings for Hanzo Base are no longer a separate package. They now ship inside the JavaScript/TypeScript client @hanzo/base as a subpath export:

import { BaseProvider, useQuery, useAuth } from '@hanzo/base/react'

The standalone @hanzoai/base-react package is retired — its hooks (useQuery, useMutation, useAuth, useRealtime, useConnectionState, usePresence, useCRDT, …) are all provided by @hanzo/base/react, which is a superset. There is exactly one React entry point now.
