zeekay b15856e41f app-server: a binary registers the extensions it adds to every thread ...

`codex_app_server::register(extension)` adds an `Extension`, a function given
the thread's registry builder and the thread manager, to every thread the
process starts; the app server installs each registered one where Kai used
to be installed by name, before Guardian's. The list is process-global
because the TUI and exec start the app server in process. With nothing
registered a thread carries upstream's extensions alone.

hanzoai/dev registers Kai's install at startup, so app-server no longer
depends on hanzo-kai, and the workspace no longer names it.
