- fetch_ci_report.js: when the top-level PR report is the only one
  discovered (no nested job report URLs), emit its failed leaves instead of
  suppressing them, so a PR URL never yields zero per-failure detail. Nested
  reports still suppress the PR-level duplicate as before.
- SKILL.md: the issue-reference verification now accepts ClickHouse's
  full-URL relationship lines (Closes/Fixes/Related: https://.../issues/<n>),
  the convention repo PRs use, not only the short #<n> form — otherwise a
  real fix PR is wrongly discarded as a prose mention.
