fix(tui/reasoning): improve collapsed-title extraction; ignore meta/preamble lines (e.g., 'Here are the key updates', 'I need to…'), treat trailing quotes/brackets when checking punctuation, and exclude ':' endings to prevent non‑titles from appearing as headings.
