- Remove tracked files that are ignored by .gitignore:
  release-assets/, artifacts/, npm-binaries/, .code-bin/, .vscode/, codex-cli/README.md.
- These are CI/build outputs or local editor configs and should not live in git.

James Peter c24adf1407 fix(tui/input): normalize KeyEvent kind on non-enhanced terminals to restore typing on Git Bash/mintty (Windows) 
This maps Release-only key events to Press when keyboard enhancement flags are unsupported, fixing unresponsive input and Ctrl+C on some Windows terminals.

Closes #18. Closes #14.

James Peter fa561b773c fix(cli/install): stop publishing 'code' bin; create wrapper only when safe and remove on collision\n\nPrevents overriding VS Code's 'code' CLI.
