feat(tui/cards): set ANSI-16 card backgrounds
...
When PaletteMode::Ansi16, render tool cards with solid ANSI backgrounds and force high-contrast text (white on dark, black on light).
Mappings:
- Auto Drive: magenta / light magenta
- Agent (read): green / light green
- Agent (write): yellow / light yellow
- Browser: red / light red
Also extend /demo to accept 'auto drive card' for quickly previewing all ANSI-16 backgrounds.

Add best-effort remote /models parity with on-disk cache + TUI preset merge.
Harden exec handling by spooling output to disk, avoiding unbounded in-memory buffering, and using best-effort Linux cgroup limits + explicit OOM surfacing so child failures don’t kill the parent.

When the provider returns usage_limit_reached, keep the turn alive by waiting until the reset window (or 5 minutes when unknown) and retrying. Apply the same behavior to compaction tasks and show a generic retrying status in the TUI.

fix(tui/history): demote earlier Answer outputs mid-turn

Add `mid_turn` to assistant history records and cells, and retroactively mark previous Answer streams as mid‑turn when a new Answer id arrives so only the final output gets gutter/bold styling.
Also upgrade legacy `gpt-5-codex` slugs to `gpt-5.1-codex`, bump GPT‑5 profile/review defaults to `gpt-5.2`, and refresh related TUI snapshots.

Add GPT-5.2 to presets/agents and upgrade prompts. Persist preferred reasoning effort separately from effective clamped effort, and keep the TUI spinner running until TaskComplete for multi-message turns.
