Add GPT-5.2 to presets/agents and upgrade prompts. Persist preferred reasoning effort separately from effective clamped effort, and keep the TUI spinner running until TaskComplete for multi-message turns.

Default Code Bridge subscription now requests all capabilities and uses a single level via subscribe; also add screenshot and command control helpers. Update tool schema/docs to match and tweak Auto Review gutter/icon spacing (⇄ glyph and extra left padding).
