zeekay 6b4a9babb0 docs: replace old Every Code logo with Hanzo Dev heading ...

Remove the every-logo.png image reference from all three READMEs
(main, dev-cli, codex-cli/.pack) and use a text heading instead.
The old logo was still showing "EVERY CODE" on the npm page.
