Major changes:
- tui_app_server merged into tui (crate deleted)
- New crates extracted: analytics, rollout, sandboxing, tools, instructions,
  core-skills, plugin, connectors, git-utils, terminal-detection
- exec-server, code-mode (V8 runtime), v8-poc
- auth moved from core to login crate
- Post-tool-use hooks, plugin marketplace improvements
- Windows sandbox hardening, shell-tool-mcp removed

Kept Hanzo binary names in macos-code-sign action.
