James Peter d43eb23e8d fix(login): remove /oauth2/token fallback; adopt upstream-visible request shape ...

- Revert 993c045 OAuth fallback and Accept header changes.
- Add originator param to authorize URL and ServerOptions.
- Add cancel flow (/cancel route) and bind retry to replace prior server.
- Keep shared HTTP client for TLS robustness; token requests now match upstream path and headers.
