- Adopt upstream changes in exec, mcp-server, and common by bucket.
- Preserve fork-specific core (codex.rs, browser/agent tools, UA/version).
- Add Prompt.output_schema; keep our Text controls; adjust tests.
- Fallback to Op::UserInput for UserTurn in exec and MCP.
- Keep ours for core codex files to preserve TUI/stream ordering.

verify.sh: pass; build-fast.sh: pass (no warnings).
