zeekay 0426fb29a1 feat: add hanzo-sse, hanzo-backoff, hanzo-md-render crates + /loop + oauth refresh ...

New crates ported from claw-code architectural patterns:
- hanzo-sse: zero-dep SSE parser replacing eventsource-stream (11 tests)
- hanzo-backoff: overflow-safe exponential retry (4 tests)
- hanzo-md-render: streaming markdown renderer + syntax highlighting + spinner (3 tests)

New core modules:
- loop_runner: /loop slash command parsing (interval + command)
- oauth_refresh: token expiry detection + auto-refresh utility

Wired:
- hanzo-backoff integrated into core/util.rs (all 5 retry call sites)
- hanzo-sse replaces eventsource-stream in client.rs + chat_completions.rs

Also merges 23 commits from openai/codex upstream (clean, no conflicts).
