zeekay c45f02f8fc refactor: consolidate to 2 workflows, add icons, KMS integration 
- Delete 4 redundant workflows (all-extensions, build-and-release,
  jetbrains-plugin, vscode-extension) — all superseded by ci.yml + publish.yml
- ci.yml: single CI workflow builds all platforms (browser, Safari, VS Code,
  JetBrains, MCP) with artifacts uploaded to dist/
- publish.yml: single publish workflow with Hanzo KMS secret gating,
  publishes to Chrome Web Store, Firefox Add-ons, Safari, VS Code
  Marketplace, Open VSX, npm, and JetBrains Marketplace
- Add browser extension icons (16/48/128px) generated from source 512px —
  fixes Safari CI build (missing Icon.png)
