zeekay f90eeeeac5 Serve an install page at each add-in host's root ...

The Sites plane reads a release back from its root before calling it
live, and neither add-in had a root document, so the office publish
answered 404 there and stopped the outlook one. index.html names the
manifest URL and the two ways to add it: one person on the web, or the
whole organization from the Microsoft 365 admin center.
