David ReissandGitHub 2339500410 Refactor membership grpc resolver (#6355) ...

## What changed?
- Allow multiple membership grpc resolvers to exist in the same process
without interfering, by including a unique string in the URL.
- Restore the safety mechanism that a GRPCResolver must be initialized
by fx before a URL can be constructed.
- Refactor some of the code in #6198 to be simpler and encapsulate
details in the membership package.

## Why?
Safety, improved organization and encapsulation, more correct behavior
of multiple services in one process.
