fix(dex): point deploys and docs at ghcr.io/luxfi/dex

Helm values, the staging and production manifests, and the operations
guide name the image .github/workflows/docker.yml publishes, so a
rollout pulls and a copied `docker pull` resolves.

deploy.sh deploys that published image; CI owns the multi-arch build.
The image table lists the tag the Dockerfile produces.
