zeekay 9ec27a3d4c database: add in-memory HeightIndex implementation
Add production-ready in-memory HeightIndex implementation with:
- Thread-safe map storage using sync.RWMutex
- Value copying to prevent external mutation
- Proper error handling with ErrNotFound and ErrClosed
- Comprehensive test suite via reusable dbtest package
- All 12 test cases passing with race detection
- Benchmarks showing ~15ns/op for Get/Has operations
