zeekay d6bea0966f feat: exchange-api GraphQL proxy for lux.exchange

Node.js GraphQL proxy that handles LUX/ZOO chain queries locally
via subgraph + Blockscout + hardcoded token metadata, and proxies
all other chains to Uniswap upstream API.

- Hardcoded token metadata for 20+ Lux mainnet tokens
- Subgraph integration for V2/V3 pool data
- Blockscout fallback for on-chain token info
- Health endpoint at /health
- Docker image: ghcr.io/luxfi/exchange-api
