CreatePayment now forwards req.IdempotencyKey to Column as the Idempotency-Key header. Empty IdempotencyKey falls back to provider.DeriveIdempotencyKey(orgID, endpoint, canonical body) so retries collapse server-side. Hermetic httptest covers caller-supplied and deterministic-fallback paths.
