CreatePayment forwards req.IdempotencyKey to Unit as the Idempotency-Key header (per docs.unit.co/#idempotency). Empty falls back to provider.DeriveIdempotencyKey. Hermetic httptest covers caller-supplied and deterministic-fallback paths.
