Thomas Leonard ff7f779e9b Replace proof-of-concept with working implementation
There are now two libraries:

- capnp-rpc defines the abstract protocol.
- capnp-rpc-lwt adds in Lwt support and capnp serialisation.

Other modules could be created for other serialisations (e.g. gRPC). The
unit-tests in `test` use in-memory data-structures, while the ones in
`test-lwt` use capnp serialisation.

The new code supports finishing questions, releasing capabilities,
error reporting, cancellation and embargoes. This is most of capnp-rpc
level 1.

Mirage flows can now be used as endpoints, and there is some very basic
fuzz testing with AFL.

Many things are still not implemented. However, it's better than the
previous system, so I'm proposing to merge it now anyway in its current
state.
