docs: add comprehensive session summary for 2025-10-06

# Development Session Summary - 2025-10-06
## Overview
This session focused on completing Phase 1 of the code-rs codebase cleanup and establishing maintenance infrastructure for ongoing development.
## Objectives Completed
### 1. Legacy Test Retirement ✅
- **Removed:** ~35,000 lines of legacy test code
- **Deleted:** 106 test files across 8 crates (app-server, apply-patch, chatgpt, cli, core, exec, login, mcp-server)
- **Impact:** Reduced maintenance burden, improved build times (1m 30s)
- **Documentation:** Created `docs/migration/legacy-tests-retirement.md` with 6-week porting plan
### 2. Upstream Crate Adoption ✅
- **Adopted:** `codex-mcp-client` and `codex-responses-api-proxy`
