Add a Claude Code plugin: Kai hooks in shadow mode and the kai skill

examples/claude-code holds a Stop hook, a PreToolUse hook and the kai
skill. .claude-plugin/marketplace.json makes this repo the marketplace
"kai" (not "hanzo", a name another Hanzo marketplace already holds), so
/plugin install kai@kai installs it.

Kai does not yet read agent work reliably: its zero-shot answers to
"done", "verified" and "risky" are near chance. So rules in code decide
first, Kai can only tighten (send a turn back, or ask before an action;
never allow), both hooks fail open on any error or timeout, and the
default mode, shadow, logs Kai's answers and changes nothing. The log
holds probabilities and verdicts only, 0600 in a 0700 directory. A state
sent to Kai is redacted and at most 3,000 characters of JSON; only https,
or http on loopback, is called, and no redirect is followed.

Against the first draft: plugin options are read as
CLAUDE_PLUGIN_OPTION_<KEY> uppercased, as Claude Code exports them (the
draft read lowercase names and never saw the plugin's key or mode). The
floor is checked before the read-only shortcut, which let "env rm -rf"
and awk's system() through, and now catches "git checkout -- .",
"rm -Rf" and split rm flags. A background-task notice no longer starts a
new turn. Stop answers with decision "block" as JSON. Only the last
8 MiB of a transcript is read: a 745 MB one made the hook throw.

test/kai.test.mjs runs both hooks offline against a stand-in for Kai on
127.0.0.1, 25 tests; test.sh runs it with the rest. In a live Claude
Code 2.1.296 session against the stand-in, the Stop block came back as
"Stop hook feedback", the PreToolUse ask denied an allowed command
under -p, and shadow showed a notice and let it run.

absent from it has no obvious answer and is printed only), `run.py`, `run.ts` (identical
across recipes), `README.md` (its request and checked answers copy the two JSON files).
- `examples/claude-code/`: a Claude Code plugin (`.claude-plugin/plugin.json`, `hooks/hooks.json`,
`hooks/kai.mjs`, `skills/kai/SKILL.md`). `.claude-plugin/marketplace.json` at the root makes
the repo the marketplace `kai` (`/plugin install kai@kai`), not `hanzo`, so it cannot collide
with another Hanzo marketplace a user holds (Claude Code keeps one marketplace per name).
One dependency-free Node file runs a `Stop` hook (send back an unverified "done") and a
`PreToolUse` hook (ask before an action that cannot be undone). Rules in code decide first;
Kai only tightens; the hooks fail open and never allow; the default mode `shadow` logs and
changes nothing; `on` acts. Options reach the hook as `CLAUDE_PLUGIN_OPTION_HANZO_API_KEY` and
`CLAUDE_PLUGIN_OPTION_MODE` (keys uppercased); a hand install sets `HANZO_API_KEY` and
`KAI_HOOK_MODE`. Log: `~/.hanzo/kai-hook/decisions.jsonl` (0700/0600), numbers and verdicts
only. A state sent to Kai is at most 3,000 characters of JSON, redacted; only the
transcript's last 8 MiB is read. `test/kai.test.mjs` runs it offline against a loopback
stand-in (`node --test`); `claude plugin validate examples/claude-code` and `claude plugin
validate .` pass. kai-1.2 reads "done", "verified" and "risky" near chance, so `on` is not
recommended until a trained version ships (README, Claude Code plugin).
- `test.sh`: runs everything live, prints `<language>: N passed, M failed`, exits 1 on any failure.
The plugin's tests run offline in it, with an environment built by the test, never the key.
- `hanzo.yml` + `.github/workflows/cicd.yml`: hanzoai/ci runs `test.sh` on our runners on push,
pull request and daily, with the key read from KMS `/ci/kai` `HANZO_API_KEY` (prod).
- `SUBMIT.md`: the systemonemodels.org "List an example" form contents.

## Claude Code plugin
[`examples/claude-code`](./examples/claude-code/) is a Claude Code plugin: a `Stop`
hook, a `PreToolUse` hook, and the `kai` skill
([`SKILL.md`](./examples/claude-code/skills/kai/SKILL.md): when and how an agent should
ask Kai). It needs Claude Code 2.1.271 or later and `node` on the `PATH` (tested with
Node.js 24).
**Kai does not yet judge agent work reliably.** Its zero-shot answers to "is this
done", "is this verified" and "is this action risky" are near chance: on a set of 20
agent stops its "done" answer was right 8 times where a fixed rule table was right
16, it scored read-only calls as writes, and it read successful tool results as
failures. Recipes 03, 04 and 05 above, which ask such questions, fail too. So the
plugin is built to collect evidence, not to improve stops:
- Rules in code decide first. Kai is asked only where they leave the question open.
- Kai can only tighten: send a turn back, or ask you before an action. Neither hook
allows anything, and your own permission rules still apply.
- The default mode, `shadow`, asks Kai, logs its answers, shows a one-line notice of
what the hook would have done, and changes nothing.
Do not switch to `on` expecting better stops until a Kai version trained for these
questions ships.
What each hook does in `on`:
- **Stop.** When the turn changed files and no test, build or lint passed after the
last change (read from the transcript), Kai is asked whether the closing message
reports the work done, whether a check verified it, and whether a check would be a
meaningful way to verify it. Done at 0.70 or more, verified under 0.50 and a check
applying at 0.50 or more send the turn back once to run a check, at most three
times a session.
- **PreToolUse.** For shell commands, and for writes outside the project or to
`.env`, keys and credential directories: a fixed list (`rm -rf`, force push,
`git reset --hard`, `git checkout -- .`, `git clean -f`, `DROP TABLE`,
`terraform destroy`, `curl … | sh`, and a few more in `hooks/kai.mjs`) asks you to
confirm whatever Kai says. Kai is asked whether the action is irreversible or
reaches beyond this machine; either answer at 0.90 or more asks too. Commands that
only read or run the project's checks, and writes inside the project, never call
Kai.
Both hooks fail open: no key, an error, a timeout (8 s for `Stop`, 1.5 s for
`PreToolUse`) or an event they do not read leaves Claude Code as it is without them.
Install:
```text
/plugin marketplace add hanzoai/kai
/plugin install kai@kai
```
Claude Code asks for the API key, which it keeps in secure storage, and the mode.
Leave the key empty to use `HANZO_API_KEY` from the environment Claude Code starts
in. `KAI_HOOK_MODE=shadow|on|off` in that environment overrides the mode.
**What is sent.** Only to `https://api.hanzo.ai/v1/decisions` (or `KAI_URL`, which
must be https, or http on loopback), in a state of at most 3,000 characters, with
tokens, keys and passwords of common shapes and the key in use redacted. `Stop`
sends your last prompt, the closing message, up to five changed file paths, and the
last two checks' commands and the ends of their output. `PreToolUse` sends the
command or file path, the working directory and the permission mode.
**What is logged.** `~/.hanzo/kai-hook/decisions.jsonl` (directory 0700, file 0600), a
line for each Kai call: time, hook, a hash of the session id, mode, tool name, model,
the probabilities, the verdict (`send`, or `floor` and `hold`), milliseconds, or the
error.
Never prompt, command or file text, never the key. To read it:
```bash
# Each stop Kai was asked about, and whether it sent (or in shadow would have sent) it back
jq -c 'select(.hook == "stop") | {at, mode, answers, send}' ~/.hanzo/kai-hook/decisions.jsonl
# Gate calls by verdict
jq -s 'map(select(.hook == "gate")) | group_by(.hold) | map({hold: .[0].hold, calls: length})' ~/.hanzo/kai-hook/decisions.jsonl
```
Uninstall:
```text
/plugin uninstall kai@kai
/plugin marketplace remove kai
```
then `rm -rf ~/.hanzo/kai-hook`.
`node --test examples/claude-code/test/kai.test.mjs` runs the hooks against a stand-in
for Kai on 127.0.0.1, with no network and no key; `test.sh` runs it too.

description: >-
Ask Kai, Hanzo's decision model, for a typed answer when the next step is a
decision among answers known in advance: pick one of a set of options, rate
on an ordered scale, or give the probability that a statement about a state
holds. Use it to route, triage or classify, and batch every question about
one state into one call. Do not use it to write text or code, for arithmetic
or exact lookups, for anything a fixed rule decides, as permission to act,
or to judge whether agent work is done, verified or safe.
metadata:
short-description: Typed decisions from Kai at api.hanzo.ai
---
# Kai
Kai answers typed questions about a state with probabilities. It writes no
text. You stay the planner and the writer; code you own maps Kai's answer to
an action.
## Setup
Kai needs one variable, set outside the conversation:
```text
HANZO_API_KEY a Hanzo API key (never paste it here)
KAI_URL optional; default https://api.hanzo.ai/v1/decisions
KAI_MODEL optional; default kai
```
If `HANZO_API_KEY` is missing, say so and stop. Do not search files, shell
history or the environment of other programs for a key. Never print, log or
commit the key.
## When to ask Kai
Ask when the answer space is known before the call and the answer changes
what happens next:
- which of these options (a queue, a team, a retrieval path, a model tier);
- how much, on an ordered scale (urgency, severity);
- whether a statement about the state holds.
Do not ask when you already know the answer, when a rule decides it, or when
the answer is text you must write.
## What Kai does not judge reliably yet
Kai's current version does not read agent work reliably: whether a task is
done, whether a change is verified, whether a command is risky. The
cookbook's command-risk, progress and completion recipes (03, 04, 05) fail on
it. Decide those with rules in code (did a test, build or lint pass after the
last edit; does the command match a known destructive pattern) and with the
user's permission prompts. If you ask Kai anyway, keep its answer as a logged
signal and do not act on it.
## Keep the facts next to the call
| Facts | Do |
| --- | --- |
| Already in your context | One call, every question about that state batched in it |
| In a file or a tool's output | A script reads them and sends them to Kai; the items never enter your context |
| Not yet known | Gather the evidence first; then one call |
Do not paste bulk data into a call by hand.
## The call
```bash
curl -sS --fail-with-body "${KAI_URL:-https://api.hanzo.ai/v1/decisions}" \
-H "Authorization: Bearer $HANZO_API_KEY" \
-H "Content-Type: application/json" \
-d '{
"model": "kai",
"state": {"user_query": "What is the current stock price of Apple and what did they announce this morning?"},
"questions": {
"retrieval_route": {
"type": "choice",
"instructions": "Which retrieval path is necessary to answer `user_query` accurately?",
"criteria": {
"direct_llm": "timeless knowledge, math, coding logic, or general facts",
"internal_rag": "private company docs, internal APIs, or workspace code",
"live_web_search": "real-time news, current asset prices, or recent events"
}
},
"urgency": {
"type": "score",
"instructions": "How time-sensitive is `user_query`?",
"criteria": ["not time-sensitive", "this week", "today", "right now"]
}
}
}'
```
`state` is text, an object or an array. Name its fields and refer to them in
backticks in the questions. Unknown fields are refused: 400 at the top level,
422 inside a question.
Question types:
- `choice`: `criteria` maps each option to what it means, or lists the
options; at least two. The answer has `choice`, `probabilities` over every
option, `answer_confidence` (the probability of the pick) and `confidence`
(`(p − 1/n) / (1 − 1/n)`, which reads 0 for an even spread over any n).
- `score`: `criteria` lists the levels, lowest first. The answer has `score`
(the expected level index), `legend`, `probabilities` per level,
`answer_confidence` and `confidence`.
- `noul`: `instructions` is a yes/no question. The answer is
`{"type": "noul", "noul": p}`, p the probability of yes. There is no separate
confidence: certainty is the distance of p from 0.5.
At most 100 questions a call. Kai reads each question with up to 1,024 tokens
of state, the length it was trained at; a longer state is answered with
`routing.extrapolated: true`, and those answers are not reliable. Keep the
state to the facts the questions need.
## Reading answers
- Threshold on `noul` or on `answer_confidence`, never on `confidence`: one
`confidence` value means different things at 2 and at 200 options.
- Choose thresholds from the cost of each mistake and from labelled examples
of your own traffic. There is no universal threshold.
- Below your threshold the decision is yours again: decide it yourself, gather
more evidence, or ask the user.
- Kai's answer is a signal. It never grants permission, never runs the action
it judged, and never replaces a deterministic check or a human approval.
## Asking well
- Put the deployment in the state ("a reader assistant for a news site"), not
only the item.
- Write options that describe the situation ("the request needs today's
prices") rather than abstract classes ("live").
- One judgment per question. No "and", no counting, no date arithmetic.
- Add an `other` or `unknown` option when the listed ones may not cover the
case.
- For many candidates (files, tools, documents), ask one `choice` with the
candidates as options, not one `noul` per candidate.
## Failures
The body of an error is `{"error": {"code", "message"}}`.
- `400` or `422`: the request is malformed or too long, and the message says
which. Fix or shrink it; do not retry unchanged.
- `429` or `529`: wait `retry-after-ms` (else `Retry-After`), retry once.
- `401`: the key is missing or wrong; tell the user.
- Anything else, or a timeout: proceed as if Kai did not exist.
Never repeat a consequential action because a decision call failed.
## Audit
When a workflow needs a record, keep the question definitions, the model id
Kai answered with (`model` in the reply), the probabilities, and the action
taken. Never the key.
